  4. Lack of Respiratory Protection (3,210 violations)
  5. Lockout/Tagout Failures (3,025 violations)
  6. Unsafe Use of Ladders (2,910 violations)
  7. Powered Industrial Truck Violations (2,365 violations)
  8. Insufficient Fall Prevention Training (2,020 violations)
  9. Machine Guarding Deficiencies (1,980 violations)
  10. Lack of Face and Eye Protection (1,590 violations)
